You have the CPT/ICD-9 codes memorized, and you’re on a first-name basis with the member services rep at the insurance company. You have prepared the documentation required by your carrier, and the big day is finally here: It’s time to submit all this information with a request for pre-approval of your gastric bypass procedure.

Increase Your Chances of Insurance Success
The checklist below can help you ensure you have obtained everything needed to increase your chances of securing preauthorizing for weight loss surgery from your insurance carrier:

1. A letter of medical necessity from your primary care physician or a medical specialist recommending you for weight loss surgery
2. Doctor’s records of office visits documenting your history of obesity (this should include documentation supporting The National Institute of Health guidelines of severe and morbid obesity)
3. A well-documented list of any serious health issues, such as hypertension or diabetes, considered a direct result of severe or morbid obesity

You may also submit the information yourself, but your request will likely be given more weight if coming from a medical practitioner.

Along with your required documents, the letter to your insurance company from your doctor will contain pertinent information to help in the pre-approval process. This correspondence should clearly identify the date, your name, and the reason for the letter (e.g., “request for preauthorization”). Your date of birth, insurance number, and group number should come immediately after this information, after which the body of the letter should follow.
Making Your Case
Within the body of the letter, your doctor should again state your name, along with your age and gender, and the long-term medical problem for which you are requesting coverage of treatmentin this case, severe or morbid obesity.

The body of the letter should also include your weight and height, and note that your body mass index (BMI) is excessive for the criteria of obesity. In addition, your doctor should state clearly that if the weight problem continues, you would suffer from serious health risks, resulting in a shorter life span. The following are other topics that should be addressed in your request for preauthorization:

• The specific period of time you have been in your doctor’s care expressly to manage your obesity.
• Details of your active involvement in recommended diet or medication remedies. Include the amount of weight lost in following these recommendations and show that you have suffered from continued weight gain after trying these programs.
• The current list of medical conditions directly resulting from weight issues, and assurance that these issues would improve greatly upon shedding the excess weight following gastric bypass surgery.
• The procedure expected to take place, the surgeon expect to perform the procedure, the address and telephone number of the surgeon or bariatric treatment center, and the expected length of hospital stay.
• A description of the surgery, including details of the benefits you seek to gain from gastric bypass surgery.

Prior to sending the request, be sure to make copies for yourself and your physician or surgeon, and attach all supporting documentation. You may also want to send your letter via registered or certified mail, so you can mark the date when your request was filed and have proof that it was received.

Is a Mother’s Love, Medicine?

Minus all meningitis thoughts. The flu symptons were strong. Headache, burning fever and sore throat. So what does any natural mother do when their offspring takes ill. They smother him or her with a lot of TLC and would walk to the ends of the earth just to take away the pain and suffering.

Is a mother’s love, medicine?

Despite all those motherly rituals mentioned above. There was no change in my nine year old son’s condition. Meningitis still so distant in my thoughts..The lucozade was poured out in pints to quench his thirst and a saturated flannel to change every minute to wipe his brow. I prayed more times to god than that of any pope for my son to pull out of this other world he had elapsed into.

A mother’s love is more of a cure than any cough medicines pills or tablets, so I thought..

Humming one of his favourite tunes as I held him up close in my arms rocking him back and too.
Not even his favourite videos could pull him out of this alien world that he now belonged to

Was a mother’s love? slowly killing her little boy.

Every mother’s worst nightmare was to befall me with the inevitable happening. Purple like bruises began to appear on his chest followed by the loss of his voice.
With trembling hands I struggled to dial 999. Please god if your up there hear my prayers, let my little boy reach 10..

When the doctor arrived he said he couldnt rule out meningitis. And so my little boy was rushed off in an ambulance at a speed that would have given Nigel Mansell a run for his money.
Thank god for flashing blue sirens.

In silence I prayed for god to give me back my son.

The curtain may well have been a brick wall that separated me and my little boy in the hospital. But he was in good hands I assured myself and that being the hands of god.

It seemed like a life time before that curtain came swishing back to have the doctor in his white coat appear before me.
I gave him lucozade I wiped his brow. I even put on his favourite videos and smothered him with all the love I could possibly give. Where did I go wrong doctor? You did nothing wrong was his reply.

Just to hear those words that there was no cause for alarm and that my little boy was going to pull through, gave me back all the strength that seemed to have drained from my body while waiting for the outcome of this nightmare.

Meningitis symptons had a strong presence the doctor said. It was a virus on the inside trying to break out.
Tears of happiness flowed like that of Niagra falls as I rushed to my little boys side.

Holding him tight kissing and cuddling him like any natural mother would do.

I had unfinished business to attend to and that was to thank the man himself up above for hearing my cry. Now to arrange a ten year olds birthday party.

The doctor approached me and asked if he could say something before I left.

Yes of course doctor?
A mother’s love is great medicine for comforting her children

Creativity Management: the Value of Frameworks

What do creativity managers do?

Replace the word management with the word optimisation.

That’s what creativity managers do: they optimise the quality of the idea pool (creativity) and the implementation process (innovation).

There are many methods of optimisation and the creativity leader must be aware of all of them, in other words, he or she must synthesise them for optimal effect.

Areas [within creativity] that need managing include motivation, organisational culture, organisational structure, incremental versus radical effects and processes, knowledge mix, group structures, goals, process and valuation.

Areas [within innovation] that need managing include idea selection, development / prototyping and the art of commercialisation.

It is worth noting that 4000 good ideas result in 4 development programs, which in turn results in 1 winner.

The value of Frameworks

Creativity and Innovation Management are complex. There is no doubt about it, and the untold number of myths floating around exacerbates this complexity.

The only way to develop a sound holistic understanding is to create a framework.

With a good framework, you should be able to easily place, like pieces of lego, anything you come up against, onto the framework. That provides context and is great for Q&A sessions.

There are at least twelve core elements to the framework, including organizational culture, organizational structure, radical versus incremental productivity and so on.

I recently had a question about the aesthetic environment and its impact on creative output. We got talking about the meaning and impact of colors. Now that’s not something I am an expert in - I can only vaguely tell you what effect yellow may have - but I just slotted it onto the framework: on the organizational culture and process (stimuli) slots and it all fit together perfectly.

It’s actually great when you come up against something you can’t place, because that may mean that you have a whole new branch to add onto the framework, which only helps to complete the picture.

Menopause–Information and Treatment Options

What Is Menopause?

Menopause is defined in the dictionary as the permanent cessation of the menstrual cycle. This occurs when a woman’s egg supply and ovarian function is depleted. It can occur naturally, or surgically with removal of both ovaries. A woman is said to be in menopause when there has been no menstrual period for 12 months. The average age of menopause is around 50 years in the US. Perimenopause refers to the time before menopause when the hormone levels begin to decline, the menstrual periods may become irregular, and fertility is reduced. Perimenopause may come before actual menopause by as much as 2-8 years, the average being about 4 years. Smoking is known to accelerate the transition phase by 2 years, as it speeds the aging process in general.

The classic symptoms of menopause include night sweats, difficulty sleeping, vaginal dryness, depression, and mood swings. One US study showed nearly 60% of US women had hot flashes as early as 2 years before menstruation stopped. Other symptoms in the perimenopause/menopause time frame include poor handling of stress, a more obvious pre-menstrual syndrome (PMS), weight gain, water retention and bloating, problems with memory. These symptoms are related to the drastic changes that are occuring in the body with reducing levels of estrogen and progesterone.

What About Hormone Replacement Therapy (HRT)?

The question as to whether or not to use HRT has now become a very complex issue. Nearly 30% of menopausal women in the US are taking hormones, commonly given to relieve the side effects as listed above. In the Women’s Health Initiative (WHI) 16,000 women were studied for over 5 years, but then the study was prematurely stopped when it was discovered that the combination of estrogen/progesterone replacement therapy resulted in an unfavorable risk to benefit analysis. There is no doubt that the therapy did benefit women’s hot flashes and risk of developing osteoporosis. Unfortunately the study revealed definite increased risk of developing endometrial (uterus) cancer, breast cancer, venous blood clots, and probably a risk of stroke and heart attacks. There was also question as to whether the use of HRT could be linked to gallbladder disease, ovarian cancer, colon cancer and even possibly an increased incidence of memory loss and dementia. Some of these risks were directly linked to the time HRT therapy was taken. In other words, the longer you take HRT therapy the higher the risk.

What About Alternative Therapy?

Several naturally occuring plant extracts have been shown to help with the symptoms of menopause and present a natural alternative to hormone replacement therapy:

Black Cohosh–studies in Germany have shown a decrease both in the number and level of menopausal symptoms.

RedClover–rich in phytoestrogens, these little molecules can be extracted from plants such as soy and have a similar structure to estrogen itself but lack the side effects of estrogen itself.

Flaxseed–Also contains a substance with weak estrogen activity.

Soy Isoflavones–also contain molecules with similar structure to estrogen.

Of course, you should talk to your doctor about your choices of therapy during the menopause and perimenopause time and what the possible side effects and risks may be to the various treatment options.

Worksite Wellness

Employers increasingly are realizing the value that worksite wellness programs deliver as an effective tool to improve employee health, increase productivity, reduce absenteeism and lower health care costs.

A 2003 report published by the U.S. Department of Health and Human Services (HHS) highlighted how important it is for employers to incorporate health promotion as part of their business strategy. The report asserts that preventable
diseases place a heavy toll on business, including lower productivity and higher health insurance costs.

The HHS estimates that $1.66 trillion was spent on health care in 2003. and it attributes a majority of those costs to chronic diseases and conditions such as heart disease, diabetes, obesity and asthma. Sadly, the money allocated for preventing or controlling these conditions is negligible.

In a recent article, American Cancer Society CEO John Seffrin reported two thirds of cancer deaths in the United States could be prevented through lifestyle changes in diet, exercise, cancer screening and “especially” tobacco use. A well-designed worksite wellness program, provided by your health benefits company, serves the best interests of both employer and employee.

Bottom line return on investment

Ron Goetzel, a nationally recognized expert in the field of health management, data analysis and applied research, said in a recent interview that with an investment of $100 to $150 per employee per year in health promotion, an employer can expect an average return on investment of approximately $3 for every $1
invested ($300 to $450 savings per employee per year).

Goetzel says, however, that these returns are not typically realized until two to three years into the wellness program.

Legislative incentives

Sen. Tom Harkin (D-Iowa) has been an outspoken proponent in seeking legislative solutions for a strained health care system.

“As a nation, we have a ’sick care’ system that is focused on helping people after they get sick, rather than a ‘health care’ system which focuses on keeping healthy people healthy,” he says.

Harkin introduced the Healthy Lifestyle and Prevention (HeLP) America Act of 2004. One of the initiatives under Title II - Healthier Communities and Workplaces, provides tax credits to businesses that offer comprehensive programs to promote employee health and grants for small business.

Worksite wellness, getting started

Implementing a worksite wellness program with your health insurance carrier can be accomplished with simple, low-cost strategies.

Offer incentives for participation.

Create a wellness campaign.

Schedule seminars on nutrition, diabetes and cholesterol.

Establish programs such as fitness, sleep diary, smoking cessation and injury prevention.

Offer chair massages or simple stretching exercises to do at the desk.

Change vending machine options to offer healthier, low-fat snacks and drinks.

Work with a health insurance carrier that can implement, manage and maintain your program. Actively promote employee participation.

A successful worksite wellness program can boost company morale, enhance productivity, reduce organizational conflict, attract superior workers and lower the rate of employee turnover. The case for establishing a worksite wellness program is well worth the effort.

The Baseball Minor Leagues - Part 1

The Minor Leagues of baseball are a professional league competing a level below the Major Leagues. They are operated as independent businesses, but the best known leagues are members of Minor League Baseball, the main organization that has agreements with the Major Leagues to operate as their affiliates. The purpose of this affiliation is to develop players that will be available to the Major Leagues upon demand.

There are twenty minor baseball leagues in operation with 246 member clubs in many towns across the United States and Canada. The Major Leagues have a 40 Man Roster; however, only 25 of those men actually play for the major league club, while the remaining 15 players usually play in the Triple-A level of the minor leagues (or may be on the disabled list). This arrangement means that the 15 players not regularly played in the majors have the opportunity to play daily and stay up to par rather than spending the season on the bench. Any minor league players that are not on the 40 Man Roster are under contract to the parent major league club, but they do not have a union. They work for a much lower salary and play somewhere between Rookie and Triple-A minor league levels.

Immortality : My ultimate aim

Immortality has always lured me. Immortality is not just living indefinitely but is to keep struggling to be alive. It means, you cannot die of natural factors but you can however die of you following beings e.g. someone switching off your simulation on a chip without you capable of doing anything coz you have not updated yourselves with the times and the environment you are living in.

But it should be noted that there is no thing such as 100% immortality. No one including the environment is fully immortal because then it would violate the law of life/energy. We will die ultimately when all the free (usable) energy of universe is used up and there is nothing to work upon.

I’m interested in achieving immortality because I want to see all that humans are capable of achieving. It would be sad for me to die before the arrival of teleportation technology, or before I taking an intergalactic tour from earth-mars-andromeda, or before achieving true Artificial intelligence or before we make ourselves habitable to silicon environments, And for the other millions of technologies and theories which are currently in distant future. I want to see them all. I know we (currently known as humans) will achieve all these one day but I also know that I might die before then. It is like going for a treasure hunt and knowing the adventure would definitely end before the goal is achieved. It is like playing a game with ‘game over’ before ‘game end’.

There are many pathways for immortality. We haven’t even achieved 0.000001% of our total potential. There is long way to go and I want to see it all. Currently, there are so many methods which describe immortality and infinite more to arrive. Some of them are:

* Running yourself into a computer simulation

* A brain transplant into your clone

* Tearing off the telomere from cells to make them immortal

* Preserving dead bodies cryogenically for future technology to make them alive (sci-fi) stuff

* Etc

Before we delve into the theory, I would like to discuss the term environment. ‘Environment’ here is discussed in the most abstract sense. The term takes meaning in context to the meaning of the sentence where it is used. It can be anything where it fits best. And yes, it is subjective here. This is because subjectivity gives more options for evolution than objectivity. More subjects more exploration.

Environment here can be earth, mars, carbon, silicon, bits and bytes, differential equations, time, EM waves, etc.

The terms ‘organism’ and ‘energy’ is also used in an abstract sense. They can mean anything what we want it to be. It should be carefully observed that in this way the terms ‘environment’, ‘organism’ and ‘energy’ are mutually interchangeable. We can name any term anything as long as it makes sense. In this sense, an organism or the energy can be earth, mars, carbon, silicon, bits and bytes, differential equations, time, EM waves, etc.
Therefore,

Environment = Organism = Energy

All these three exist for the benefit of each other or in other words, they depend on each other for its existence. They exist mutually.

Now we take a look at the organism. But please note that you can freely replace the term ‘organism’ with ‘environment’ or ‘energy’ in any of the text below or above. For example, if an organism lives or it has a life by exchanging energy with stale or nonliving organism, we can very well say that the environment lives but not the organism. Or that energy lives on organism via environment as a medium.

Why do we live? Why do we come into existence? Basically, it is the environment which drives the life. As with every system, the environment wants to decrease its free energy. That is why it gives birth to life (it is a part of evolution). When the life comes into being, then the search for ultimate energy harvester starts. This is called evolution. As the complexity in life emerges, the organisms become better energy harvesters. They take energy from the environment, use it thus converting the free energy into unusable energy which is what environment wants. Environment makes us die because it does not want to halt the process of evolution. Evolution is very much necessary for environment because it is the process by which new and better energy harvesters comes into being. If an organism lives forever he possibly will trap the resources for himself which might lead to better organisms or which other organisms can use better. Thus it halts one path for evolution to occur and environment certainly does not want it. To become immortal, an organism needs to do one of the following:

* Fool environment into thinking that it has evolved

* Constantly changing the environment (from carbon to silicon to bits n bytes): thus making itself new (to environment) and thus giving time to environment to analyze its feasibility.

* By constant hopping around new environments, each time it gets into a new environment it gets more time to live

* Actually evolve into the existing environment

* Create a new environment in which it is the ultimate organism. By ultimate organism, I mean an organism which is the best harvester of the its current environment.
